PICKUP OR DELIVERY SERVICE - POINTS IN ARIZONA

When the carrier or its agent performs Pickup or Delivery Service at the following points in Arizona, the shipment will be subject to an additional pickup or delivery charge of $10.00 per cwt, subject to a $120.00 minimum charge, $500.00 maximum charge, which shall be in addition to all other applicable charges, as follows:
